Program Guidelines

• Patient portion of the bill is due at time of scheduling
• Program cannot be used in conjunction with
  other dental plans or programs
• No refund of premiums will be issued if participant decides not to utilize dental plan
• Program is only valid at Serenity Dental
  and Weeping Water Dental.
• Program cannot be used for referral to specialist or for hospital care
• Program cannot be used for treatment for which, in the sole opinion of our doctors, lies outside the realm of their capability
